Incident data aggregation in a behavioral health and substance use facility is an essential process that supports safety, improves patient care, ensures regulatory compliance, and drives continuous quality improvement. This information enables a more effective, efficient, and compassionate approach to managing patient care.
Incident data aggregation is a valuable resource for leadership to access. We strongly encourage clients to use Kipu Compliance's Incident Reporting forms to pull the data provided in the Incident Data Aggregation.
- Click Reports on the navigation menu on the left side of the page.
- From the drop-down list, select All to view all available reports.
- Locate the Incident Data Aggregation report, using the search bar if needed, and click on it.
- By default, the date range will display the information incidents in the last 30 days. To change the date range, click the calendar icon and select your preferred date range.
- After making a change to the filters, click the Filter Results button.
- The report will refresh with data matching your filter selections. Scroll down to review the aggregated data for the selected date range.
- At the bottom of each section, the source of the information is displayed.
- Click the source link (for example, Incident Reports) to view all related incident reports.
- Review the data in this report, which includes the following information:
- Dates and times of incidents
- Internal incidents by category
- Root Causes allow you to identify common causes and implement preventative measures.
- Dates and times of incidents
