Completing the BHT Shift Report

BHT Shift Reports are a critical communication tool used to ensure continuity of care, support staff transitions, and maintain clinical and regulatory compliance within behavioral health settings. Behavioral Health Technicians (BHTs) complete these reports at the end of each shift to document key activities, observations, and any notable events.

Let's walk through the steps to complete a BHT Shift Report in Kipu Compliance.

Required Permissions: Your user role must have been granted permissions to create the BHT Shift Report form before you may complete the following steps.

Completing the BHT Shift Report

  1. Click the Search icon in the top-right corner of the User Dashboard.
  2. In the Find in Kipu Compliance pop-up begin typing the name of the form and then select the correct option from the drop-down.
  3. Click Add New to open a blank shift report.
  4. Fill out the following details in the top section of the form. Required fields are indicated with an asterisk(*).
    • Enter the Date of the shift.
    • Enter your Name in the Employee field.
    • Specify the Address or Building Name where your shift occurred.
    • Select any Shift Partners who worked with you.
    • Choose your shift from the Select Shift drop-down menu.
  5. Under Incidents, choose Yes or No to indicate if an incident occurred.
    • Click Add Another to enter multiple incidents.
    • If an incident report has already been filled out, select Yes, then enter the Incident ID Number.
    • If you haven’t filed a report, select No and then click the link in the prompt to create one directly from the form.
    • Important: All incidents must be reported. If you think an incident occurred and it has not yet been reported, notify your supervisor immediately.
  6. Indicate if any of the following issues occurred by selecting Yes. If Yes is selected, a section will appear below where you can elaborate on the issues that occurred on your shift. All fields required.
    • For Behavioral, Medical/Nursing, and Clinical issues, specify the issues using the checkboxes, and enter the client name (first name, last initial) along with a description of what occurred in the textbox. Click the Add Another button if more than one checkbox was selected.
    • Maintenance Issues: If Yes is selected, the Maintenance Request Form link will appear. Click on it to complete the form.
  7. In the Shift Tasks section, check off all tasks completed during your shift.
  8. Under the housekeeping section, list any vacated rooms that need to be cleaned.
  9. Type in the correct answers for the following fields. Fields indicated by asterisks (*) are required.
    • MAT Clients?: If the answer is yes, the following section will expand allowing you to enter the names of each of the clients currently on MAT therapy
      • Note: Only list one patient per line. Use the Add Another button to enter additional patients.

    • Do you have any Clients on Suicide Precaution?: If yes, the following section will expand allowing you to enter the names of each of the Clients on Suicide Precaution:

  1. Add any required notes in the Shift Notes section to assist the incoming staff.
  2. Click Save to complete the form.
  3. Look for the green confirmation banner indicating the form was created successfully.
  4. If changes need to be made, click the blue arrow on the right side of the screen and select Edit from the drop-down.
